We require all office visits co-pays to be paid at time of service.  If you have surgery, we require all surgery deposits be paid to our office 3 days prior to surgery.  We would be happy to work with you with a payment plan prior to your surgery. We honor Visa, MasterCard, and Discover.  We take cash, money orders, and cashier’s check, but no longer accept personal checks.
